Your First Appointment

1. Have your Oregon Health Plan ID number ready
2. Call MultiCare Dental at 503-988-3711 (in Portland) or 1-877-350-3733 (toll free)
3. Arrive 15 minutes early. If you cannot keep your first appointment, call to reschedule at least 24 hours prior to your appointment time.

After Your First Appointment

Call your clinic directly to make follow up or changes to your appointment(s). If you cannot keep your appointment, call the clinic at least 24 hrs prior to your appointment time.

Restrictions may be applied to members who fail to keep their appointments. Inform any change of address or telephone number to the clinic staff when you check in for your appointment(s).
